Only one can be equipped at a time, and neither one changes a single rule, stat, or mechanic. If your crewmates are voting you out, a brain parasite on your back is not going to save you. Among Us itself is a well-established social deduction game built around four to ten players working out who the impostor is before the body count climbs too high. Tasks run as minigames, meetings turn into argument sessions, and the whole thing lives or dies on the people in your lobby. The base game carries a 92% positive rating across well over 700,000 Steam reviews and an 85 on Metacritic - that reputation belongs to the core experience, not to this DLC, which has its own much smaller review pool of around 125 opinions sitting at 91% positive.
