You are based in Pelican City, a fictional West Coast US setting split across three districts that unlock as you level up your paramedic career. Each shift starts with a dispatch call, sends you into traffic with sirens active, and drops you at a scene where you need to quickly read the situation. The complaint variety is genuinely broad: 36 different incident types ranging from minor lacerations to third-degree burns from an exploding petrol station, plus large-scale catastrophe events per district that throw multiple casualties at you simultaneously and force real triage prioritisation. That is the part of the game that works. When you are cross-referencing symptoms on your in-game tablet, selecting from 17 medical instruments, and deciding whether your partner paramedic should start CPR or adjust the patient's temperature during transport, the pressure is real and the feedback loop is satisfying. The part that does not work as cleanly is how the game resolves that pressure: through minigames. Starting an IV? Sliding bar. Wrapping a wound? Another timed prompt.
