Transformation Tycoon

Before this clinic pays you back, it wants roughly two hours of patient queue-watching and a research tree you cannot rush. That is the deal Transformation Tycoon offers, and the adult premise (it is firmly 18+) is honestly the least interesting thing about it. What you are actually running is a small facility sim: clients arrive with requests, you assign staff, you unlock new transformation techniques through research, and you sink profits into clinic upgrades that widen what you can accept. The one loop I liked more than expected is staff progression. Employees have their own wishes, and granting them the transformation they want doubles as a retention mechanic, so your payroll and your research queue argue with each other in a way that produces real decisions. The ceiling is low, though. Fame and infamy nudge which clients show up but never force a genuine strategic pivot, and once the research tree is mapped there is no scaling pressure to keep the late game tense. It is singleplayer only, with no mod support to extend it. The 85 percent positive rating across a small review count reads accurate to me: people who want this specific fantasy plus a competent tycoon skeleton are getting exactly that, and nobody is pretending it competes with a proper sim. Go in expecting an evening or two, not a campaign.
