Hard strips lighting down to your flashlight only, which genuinely changes how you read the environment. Impossible mode gives Marny omniscient player tracking, turning the run into a pure memory and routing exercise. The AI does have a documented inconsistency problem, where Marny can lock onto a hiding spot and wait far longer than intended, which breaks tension in the wrong direction, but it does not happen on every run.
